Welcome to The Unit for Laboratory Animal Medicine (ULAM) Ordering home page. The information provided here is to assist the University of Michigan research community, and those investigators who use animals in teaching and research.

The Order Form should be completely filled out using Adobe Acrobat Version 3.0 or later. The form should be printed so signatures and areas that need to be circled can be applied and faxed back to the (ULAM) Office, or returned by hand.
